Integrated Pathways is a private Occupational Therapy and Manual Therapy Clinic. Our focus is to provide comprehensive and effective treatment and deliver interventions that address the root of each issue.

Our skilled therapists work with you to create a treatment plan which fits one’s individual needs. We can also be a part of your health care team, working with physicians and other health care providers who are important to you.

Services & Costs

Kimberly Clark, OTR/L, RCST®:

• A private one hour Integrative Bodywork session with Kimberly is $120.
  •  If you wish to submit to your insurance for reimbursement, an additional $150 charge is necessary for a formal OT evaluation which would be necessary on your first visit.  You will need to let me know BEFORE this initial visit if you plan to seek insurance reimbursement for your payments.  *See more information below
  • Some insurance carriers now include massage benefits which you can use with our office.
  • If you have an HSA, FSA or Medical Reimbursement account, you can access those funds for treatment. 
  • Frequency Specific Microcurrent is available during your bodywork session for an additional $10; or as a stand alone treatment for one hour at $100 or an hour and a half for $150.
  • Lisa Nejezchleba, LMT:
    • A one hour massage is $75.00. 
      

    • *Insurance/Private Pay

      Kimberly Clark, OTR/L, RCST® is not a participating provider with insurance companies.  You may be able to access your Out-of Network Occupational Therapy benefits (which often fall under a separate deductible). However you will be responsible for payment of services at the time of treatment and will be provided with a superbill receipt which you can submit to your insurance company for reimbursement to you.   In order to access your Out-of -Network OT benefits, you will need to provide this office with a diagnosis from your Doctor and inform your therapist at the start of treatment in order for insurance based documentation can be completed on your behalf.  Retroactive documentation can not be provided.

Payments can be made by check, cash, and credit card.

Receipts provided upon request.

NON HSA Credit cards are subject to a  processing fee.
